DEACON/PASTORAL CARE

Use this form to communicate information or request assistance from the Deacons and Pastoral Care Team regarding:
- Hospitalization of a congregant
- Death of a congregant
- Funeral or memorial service needs
- Shut-in needs
- Meal support for an individual or family
- Other individual, family, or community situations we should be aware of.

REQUEST CARE

CARE APPOINTMENT
Care Appointments are available to congregants experiencing high distress and spiritual/emotional anguish. During these 1-hour sessions, our trained care providers listen to your story, triage needs, deliver spiritual and emotional support, assist with immediate stabilization, and reinstill hope as we put a plan together for helpful next steps.

BENEVOLENCE APPLICATION

FINANCIAL ASSISTANCE
The Benevolence Team provides members of Emmanuel and the community with relief from short-term financial burdens. If approved, we make monetary payments directly to the creditor/provider to help individuals move back toward financial stability.

WEDDINGS & PRE-MARITAL
We believe marriage is a sacred, lifelong covenant between one biological man and one biological woman as designed by God, and we would be honored to help you prepare for this commitment. Please complete this form with your fiancé to confirm your understanding and agreement with Emmanuel’s wedding policies, whether your service is hosted on-site or officiated by an Emmanuel pastor.

GRIEFSHARE GROUP
GriefShare is a 13-week support group program that helps people cope with the death of a loved one through video seminars, small group discussions, and workbooks, offering a safe space to find comfort, clarity, and hope by learning about the grief process and sharing experiences with others who understand.
